Athletic Paperwork
Any student who plan on participating in any sport on
any level at St. John Bosco must have the following paperwork on file
in the athletic office.
1. A physical card (signed and stamped by a licensed physician)
2. An emergency treatment card (filled out on both sides)
3. A Sports Warning Statement
4. CIF Code of Ethics
5. SJB Athletic Contract
6. Student Parent Guide Statement
You may pick these items up from Mrs. Pantuso in the athletic office.
A student will not be allowed to participate in any practice, try-out
or competition until ALL SIX items are received in the athletic office.
Please do not wait until the first day of practice to take care of the
above. The items listed above are good for the entire school year.
