The formal application process is in two parts:
Part I: The following forms and information along with a $5 check made
payable to St. John Bosco High School must be submitted by February
20, 2004.
A. Copy of 2003 filed 1040 Income Tax Form or Welfare Statement
B. Copy of your 2003 W-2 Form(s) and/or Welfare Statement
C. Signed statement (included in application packet) giving St. John
Bosco H.S. permission to obtain additional information from credit reporting
agencies during the review process.
D. Complete the Information Form included in application packet.
These items must be submitted to St. John Bosco High School by the above
February 20, 2004 deadline.
Part II: Complete and mail the SSS form (available at
the February 2, 2004 meeting) with the stated fee by February 20, 2004.
Any incomplete application, Part I and Part II above, or late application
after
February 20, 2004 will be given second consideration.
Applicants will be notified of acceptance and program by April 29, 2004.
